• Arrive to your appointment early and receive one (1) entry.

That's it! Whether it's a routine check-up, consultation, filling, etc. all you have to do is show up early for your appointment! 
All entries will be placed within a box and Dr. Long will draw a name at the end of the month.

Make sure to give us the best contact information possible because if we cannot reach the winner after 5 days we will perform the drawing again.
